Student Health Requirements
Student Health Information for the 2016-2017 School Year
All students must have proof of proper immunizations and a physical before entering school; a lead blood screening for all pre-school and kindergarten students is also required. Eye exam is due by October 15 of the current school year for Kindergarten students and any students entering Illinois schools for the first time. Kindergarten students and new students to the district will need a court house birth certificate, and proof of residency. Kindergarten, second, and sixth grade students must provide proof of dental exam by May 15 of the school year.
-All students entering, advancing, or transferring into 6th, 7th, 8th, 9th, 10th, 11th, or
12th grades will be required to show proof of one dose of the Tdap vaccine (combined tetanus, diphtheria, acellular pertussis) regardless of the interval since the last DTaP, DT, or Td dose.
-K-12 students must show proof of two (2) doses of the measles vaccine, rubella vaccine, and mumps vaccine
-Students entering Kindergarten, 6th, 9th, 10th, and 11th grades will need to show proof of two (2) doses of the varicella vaccine.
-Pre-K, K, 6th, and 9th grades must show proof of school physical on State of Illinois physical form dated within the year prior to the first day of school. (Dated 8/22/15 or after for the 2016-17 school year)
Meningococcal Conjugate Vaccine requirement for students entering 6th or 12th grade
6th grade: one dose of MCV4 at school entry;
12th grade: two doses of MCV4 at school entry; 2nd dose must be administered on or after 16th birthday
Minimal intervals allowed between doses and other options for proof of immunity: The 1st dose received on or after the 10th birthday, and the 2nd dose received, on or after the 16th birthday, at least eight weeks after the 1st dose. If the 1st dose is received at 16 years of age or older, only one dose is required.
All students participating in sports must have an athletic physical.
In August 2013, Public Act 098-0305 took effect. This act requires the IHSA to be given a CPR training video to post on its website so that staff at IHSA-member school can watch it. The act also indicates school districts shall notify parents and encourage them to view the video, too.

School time coverage (accident only)
Low plan: $15.00
Middle plan: $36.00
High plan: $66.00
The school time plan provides coverage while an insured student is in or on school premises during the days and months when school is in session; traveling directly to or from their residence and school in a vehicle supplied by the school; and participating in or attending activities sponsored solely by the school that are continuously supervised by a school official or employee. This also includes supplied and supervised travel directly to and from such sponsored activities; and school sponsored and supervised sports, excluding ninth-, tenth-, eleventh-, and twelfth-grade interscholastic football.
Around the clock coverage (accident only)
Low plan: $68.00
Middle plan: $144.00
High plan: $266.00
Around the clock coverage applies 24 hours a day, whether school is in session or not. The insurance is provided from the effective date of the insure student’s coverage to the termination date on the policy. This coverage includes school sponsored and supervised sports, excluding ninth-, tenth-, eleventh-, and twelfth-grade interscholastic football.
Interscholastic football coverage
Provides coverage for ninth-, tenth-, eleventh-, and twelfth-grade interscholastic football only.
School time and around the clock coverage is not included with this plan option.
Summer day camp/Off season conditioning
Low plan: $11.00
Provides coverage during school sponsored and supervised summer day camps that are conducted on school premises. Off season conditioning provides coverage when under the direct supervision of the coach or a trainer for conditioning and weight training for interscholastic sports which take place at a designated facility on the premises or in close proximity to the school. It does not provide coverage for play or practice involving bodily contact of any sport. This coverage ends the first day of official practice or the first day of school.
