All students must have proof of proper immunizations and a physical before entering school; a lead blood screening for all pre-school and kindergarten students is also required. Eye exam is due by October 15 of the current school year for Kindergarten students and any students entering Illinois schools for the first time. Kindergarten students and new students to the district will need a court house birth certificate, and proof of residency. Kindergarten, second, and sixth grade students must provide proof of dental exam by May 15 of the school year.
All students entering, advancing, or transferring into 6th, 7th, 8th, 9th, 10th, 11th, or 12th grades will be required to show proof of one dose of the Tdap vaccine (combined tetanus, diphtheria, acellular pertussis) regardless of the interval since the last DTaP, DT, or Td dose.
K-12 students must show proof of two (2) doses of the measles vaccine, rubella vaccine, and mumps vaccine
Students entering Kindergarten, 6th, 9th, 10th, and 11th grades will need to show proof of two (2) doses of the varicella vaccine.
PreK, K, 6th, and 9th grades must show proof of school physical on State of Illinois physical form dated within the year prior to the first day of school. (Dated 8/22/15 or after for the 2016-17 school year)
Meningococcal Conjugate Vaccine requirement: Students entering 6th or 12th grade
6th grade: one dose of MCV4 at school entry; 12th grade: two doses of MCV4 at school entry; 2nd dose must be administered on or after 16th birthday
Minimal intervals allowed between doses and other options for proof of immunity:
The 1st dose received on or after the 10th birthday, and the 2nd dose received, on or after the 16th birthday, at least eight weeks after the 1st dose. If the 1st dose is received at 16 years of age or older, only one dose is required.
